He married Betty Laura (Keeton) on Saturday, June 22, 1968 in Chicago, IL. Mr. Slavik was a Special Equipment Techn at Loyola Hospital, Maywood, IL and a U.S. Navy Veteran He is survived by his wife, Laura Slavik of Ramsey, IL. One Son: David Slavik & Janelle, San Francisco, CA, Two Daughters: Marcy Mikalik & Jim, Oconee, IL and, Sherry Slavik & Ted Shokes, Bolingbrook, IL . Mr. Slavik is also survived by 3 Grandchildren: Karina, Jimmy & Michael and One Sister: Rose Harrison of AR. He was preceded in death by his Parents, 2 Sons and 1 Brother
