He is survived by his mother: Eula Haslett and Lyle of St. Elmo; daughter: Sarah Johnson and Jason of Mich.; Brother: Art Stephens and Malia, St. Elmo, IL; and Sister: L.Louise Forbus and Gary, Fillmore, IL; Grandchildren, Austin, Richard, Heaven and Destiny. He is preceded in death by: his father, Joseph Stephens and step-father, J.D. Johnson.
