He was born May 26, 1964, in Vandalia, IL, the son of Jackie D. Cook & Joyce Ann (Washburn ) Wise. Married Janet Ruth Nichols May 10, 2009 in Vandalia, IL. He was formerly employed at Fedders, Effingham, Fayette Service Co. and Wal Mart, Vandalia, IL. Kevin was an avid hunter and trapper.

He is survived by: Wife - Janet R. Cook, Vandalia, IL; Father - Jackie Dean Cook, IL; Mother - Joyce A. Wise & Bob, Mulberry Grove, IL; Sons - Keith Cook, Josh Sidwell & Lacey, Jacob Cook & Tessa, all of Vandalia, IL; Daughters - Brittony Cook, Brownstown, IL, Kyah Cook, Breeze, IL, Sierra & Raven Cook, at home; Grandchildren - Kaleb, Kyli, Hadin, Hunter & Landen
